Admission Procedures

Please follow the procedures for admission as below after you finish emergency treatment.

1. Fill out the Medical Treatment Form and submit it to Emergency Reception.

2. Fill out the Admission Forms (Commitment form & Consent form) and submit them to the Admission Counter within 3 days of the admission date.

◆ What to bring
Hospital gowns are provided at a cost of 76 yen per 1 day, but you can bring your personal items which make your hospital stay as comfortable as possible.
If necessary, you may wish to bring your own:

  • Robe and underware
  • Slippers, socks
  • Box of tissues
  • Towels
  • Toiletries: Toothbrush, Toothpaste, body soap, shampoo & conditioner, comb, personal-care items
  • Mugcup

*You can buy some of the above at the convenience store in hospital.
*Also, TV and refrigerator are available at a cost of 216 yen per 1 day.

Meals

During your stay, meals are served as follows.
【Meal times】  Breakfast 7:30   Lunch 12:00   Dinner 18:00

*In case of non-insurance, you have to pay the full amount of meals.
(2,211 yen/3 times a day)
*While staying at hospital, patients often have special dietary requirements or restrictions for their diseases. Please refrain from buying junk food or snack without doctor’s permission.

Notes

  • Lights-out is at 9:30pm.
  • Smoking is NOT allowed in hospital.
  • You can do the laundry on your floor. It costs 100 yen at 1 time, but we don’t provide the laundry detergent.
  • Taking shower is limited while you stay in hospital. Please do not use it without doctor’s permission.
  • Please ask staff when you would like to go out or stay out.
  • We may charge you for replacement of materials missing, lost or damaged.
  • Routs for emergency exits are displayed in each patient’s room. Please make sure the routs to evacuate.
  • Do not use the elevator in emergency.
